Effect of Integrated Lumbar Scoliosis Correction Program on Pain and Posture in Chronic Lumbar Radiculopathy

Abstract
Lumbar scoliosis is a three-dimensional spinal deformity that alters spinal biomechanics and may contribute to chronic lumbar radiculopathy, leading to persistent pain and impaired postural control. This study evaluated the effectiveness of an Integrated Lumbar Scoliosis Correction Program (ILSCP) on pain and postural stability compared with conventional physiotherapy. Forty participants aged 25–55 years with clinically confirmed chronic lumbar radiculopathy associated with lumbar scoliosis were randomly allocated into two groups. Group A (n=20) received ILSCP, consisting of postural awareness training, scoliosis-specific corrective exercises, neuromuscular control training, and functional postural correction. Group B (n=20) received conventional physiotherapy including mobility exercises, stretching, and strengthening. Pain intensity was assessed using the Numerical Pain Rating Scale (NPRS), while postural stability was evaluated through posturographic sway velocity measurements. Baseline demographic and clinical characteristics, including age, sex distribution, Cobb angle, and duration of radiculopathy, were comparable between groups. After six weeks, Group A demonstrated a reduction in NPRS scores from 7.42 ± 0.88 to 3.36 ± 0.92 (p<0.0001), whereas Group B improved from 7.38 ± 0.91 to 4.89 ± 1.04 (p<0.0001). Post-treatment pain scores were significantly lower in Group A than in Group B (p<0.0001). Sway velocity decreased from 12.84 ± 2.96 mm/s to 7.12 ± 1.85 mm/s in Group A, compared with 13.10 ± 3.14 mm/s to 10.68 ± 2.74 mm/s in Group B. The Integrated Lumbar Scoliosis Correction Program was more effective than conventional physiotherapy in reducing pain and improving postural stability in individuals with chronic lumbar radiculopathy associated with lumbar scoliosis.
